A horse who trips in front, travels badly and nods intermittently is rarely telling a story about one leg. The head nod is not an answer. It is a symptom of a horse redistributing load away from something that hurts, and read as a diagnosis it sends practitioner after practitioner into the limb that nodded, while the actual problem sits somewhere nobody has been asked to look.
Not badly. Not every ride. Enough that she had stopped taking him down the lane, and enough that she had mentioned it, in passing, to most of the people who came onto the yard. It had never made it onto anybody's notes.
By the time I saw him he had been through six practitioners across twelve months, vets and physios, all competent, all doing the best job available to them with the tools they had. He had over five thousand pounds of investigation behind him and no conclusive answer.
"He's a nightmare to travel. He scrabbles about. He nearly went over last time."
She said it the way you would mention the weather. An afterthought, offered at the car door, not part of the problem.
Nobody had asked.
Why twelve months of effort produced no answer
Every practitioner who comes onto a yard answers the question they were asked.
The saddle fitter answers a saddle question. The dentist answers a teeth question. The physio answers the question of what is tight this week. The vetting answers whether the horse was fit to buy on the day it was performed.
Each of those answers can be entirely correct, and the horse can still deteriorate, because nobody in that sequence is being paid to stand back and ask the only question that resolves anything. Not "is he lame", but "why is he moving like this".
This is the gap that costs owners years, and it is the reason so many horses accumulate effort without accumulating progress. It is not a shortage of expertise. It is a shortage of anybody joining the expertise up.
What a head nod actually tells you, and what it does not
A head nod is a weight shift. The horse lifts his head as the sore limb lands, to move load off it.
That is all it is.
It does not tell you what hurts. It does not tell you where. It cannot tell you why. And in the horses that concern me most it does not appear at all, because a horse who is uncomfortable in both front feet has nowhere to shift the load to, and a horse whose problem sits behind or through the body will often stay perfectly level while quietly falling apart.
A head nod is a receipt. It is proof that a transaction has happened somewhere in the body. Reading it as a diagnosis is like reading a receipt and concluding the problem is the till.
There is a second issue, and it is structural. A trot up happens on hard, flat, straight ground, for roughly twenty seconds, on the one surface a horse finds easiest to bluff his way across. It is a single moment on a single surface, and it is being asked to answer a question about a horse's entire way of moving.
The thoracic sling: why a horse has no collarbone
Here is the piece of anatomy that explains almost every horse in this article, and it is in every textbook any of us have read. We simply do not treat it as though it were true.
A horse has no collarbone. There is no bony joint anywhere between the shoulder blade and the trunk.
Instead, the entire ribcage is suspended between the two shoulder blades in a sling of muscle and connective tissue. The serratus ventralis fans from the inner surface of each shoulder blade onto the ribs and the lower neck vertebrae. The pectorals strap the limbs in from underneath. Rhomboideus and trapezius hold the blade up from above.
That sling carries roughly sixty percent of the horse's bodyweight before a rider sits on it. On every stride it also has to decelerate the whole trunk as the forelimb takes the ground, which means the loads passing through it are considerably higher than the horse's static weight.
The forelimb is not attached to the horse. It is suspended from him. Everything the front end does well depends on soft tissue holding position under load, and soft tissue is exactly what a lameness workup is least equipped to see.
The front of that sling anchors at the base of the neck, where the neck meets the chest. It is also the region the nerve supply to the forelimb emerges from. It is, in other words, a small and heavily loaded junction that a great deal depends on, and when it stops moving freely, the sling loses the thing it hangs from.
The horse cannot reliably stabilise his front end, so the foot does not land quite where he expects it to. That is the trip.
He cannot brace himself when the ground moves underneath him, so he scrabbles and loses his feet in a moving trailer. That is the travelling.
And he loads his front feet unevenly for months, which slowly rewrites hoof balance from underneath. That is the farrier quietly changing what he does every cycle, without anybody connecting it to something happening above the knee.
The nod, when it finally appears, is the last of these to arrive, not the first.
What this looks like in a dressage horse
They arrive as a horse who is honest one day and inconsistent the next. As a downhill feeling that no amount of correct riding shifts, because the mechanism that lifts the thorax between the shoulder blades is the mechanism that is failing. As a horse who struggles to sit and take weight behind, since a front end that cannot support itself has no reason to give the hindquarters the space to come under.
They arrive as a left rein that never matches the right, and as lateral work that stalls in the same place every season.
And they arrive, most often, as behaviour. Ears back at the stable door. Tension every morning at tacking up. A horse everybody has agreed is simply sharp, or grumpy, or not a people horse.
Behaviour is data. A horse who braces when the saddle comes towards him is not describing his personality. He is describing a front end he cannot trust.
For a rider who is putting in the hours, this is where effort and outcome come apart. The work is correct. The schooling is consistent. The results are not, because no amount of training will resolve an architectural problem, and the horse has been reporting the architecture for months in a language nobody has been reading.
The horse described at the start of this article presented with four separate findings.
An intermittent head nod in front. Tripping in front, watched for a year, never recorded. Hoof balance that had shifted steadily over the same period. And an inability to stay upright in a moving trailer.
Six practitioners. Four findings. None of them had ever been written in the same sentence.
I assessed him standing and moving, on more than one surface, and then took him through one of my assessment frameworks. It led me to the shoulder girdle. He was unstable through it and blocked where his neck meets his chest, at the anchor point of the sling holding his entire front end up.
After the session I rang his farrier, because the feet were never the villain in this story. The feet were the record. Twelve months of uneven loading writes itself into a hoof, and freeing the front end without changing what happens underneath it simply returns the horse to the same pattern within a few cycles.
Four weeks later his owner messaged me. The tripping had stopped. He was travelling square instead of scrabbling. And for the first time since she had owned him, he was friendly to tack up in the stable.
Twelve months and five thousand pounds on one side. Four weeks on the other. The difference was not skill, and it was not equipment. It was the question.
How to check your own horse on two surfaces
The most useful shift available to any owner is not another practitioner. It is a better question.
"Is he lame" is a closed question. It returns a yes or a no, and once it has returned a no the conversation ends, usually with the owner feeling slightly foolish for having asked.
Starting to answer it costs nothing. Film the horse on two different surfaces, hard ground and a school, on the same day, with the same handler, on the same rein, a minute on each. Then watch the two clips back to back.
There are three things I look at when I do this.
Stride length. Whether he covers the same ground on both surfaces. A horse who shortens on the hard is reporting that landing costs him something.
Head and neck position. Where he carries them, and whether that changes between the clips. A head that lifts, or a neck that sets hard, is a horse propping up a front end he cannot trust.
The wave through the back. The spine should move like a wave, swinging softly through the horse as he travels. When it flattens and goes rigid, he has stopped moving and started holding himself together.
One of the three changing between surfaces is worth a conversation. Two of them changing is worth a proper look.
A horse who looks sound on one surface and not on the other is not sound. He is coping, on the surface that allows him to.
An owner does not need to be able to name what she is seeing. She only needs to notice that it changed, and to refuse to let that go unexplained.
Horses do talk. It is just not in English.
Finding the shoulder girdle in that horse took one appointment. Holding the change took daily work, and daily work is the part I am never there for.
